Appointment services at SEVA
All patients must be 18 years of age or older.
ONLY PATIENTS will be allowed inside the clinic doors. Only exception is for translation assistance.
APPOINTMENT CANCELATION POLICY:
Patients are required to cancel appointments at least 48 hours in advance.
After two appointment cancelations patients will no longer be provided services at the clinic.
We have a large population of community members in need of medical care and every time a patient cancels that is an opportunity for another person in need to obtain care to be wasted.

How to apply for an appointment.
- On the calendar below click on date you would like to request an appointment.
- Click on APPOINTMENT FORM link.
- Answer all questions on the form, then hit the SUBMIT BUTTON
- Before the time of your appointment, you should receive a confirmation call from us telling you when exactly you have been scheduled to come in. If you have not received a call, please call us to make sure you are scheduled to be seen.
Please make sure to input the correct information, as you can only complete the form once.
There are a limited amount of spots every week for patients to come in, so if you are you are unable to fill out the appointment form, you may call us at 281-942-7382 and we can put you on a waitlist for your preferred week.
